intentional deviations from safety procedures remain the main causes of near misses, while fires and enclosed space incident are the main incident causes. Tragically, over 50% of reports submitted to CHIRP resulted in one or more fatalities, almost all of which were the result of entry into enclosed spaces which had not been sufficiently ventilated beforehand. broad spectrum of the international movement of cargo.&nbsp; A key goal is to create an awareness throughout the freight industry, amongst operators, regulators and policy makers as to practical and effective measures to improve safety.\\u201d<\\\/p>\\n<!-- \\\/wp:paragraph -->\\n\\n<!-- wp:paragraph -->\\n<p>A fundamental part of the group\\u2019s output will be publications, an aim that is close to the heart of Deputy Chair of CINS, Dirk Van de Velde, \\u201cAs an example of where immediate attention is required, container ship fires are high on the list,\\u201d he said.&nbsp; \\u201cThe combined knowledge, experience and database resource of the signatories to this MOU, managed in a coordinated manner, have massive potential to leverage change in safety processes.&nbsp; We practical changes that will alleviate such dangers, the MOU calls for coordinated efforts both on regional and international issues of common concern and engagement with relevant regulatory bodies Tragically, over 50% of reports submitted to CHIRP resulted in one or more fatalities, almost all of which were the result of entry into enclosed spaces which had not been sufficiently ventilated beforehand. The next leading cause of reported deaths was people unintentionally falling into the water (MOB) who were not wearing life-jackets.<\\\/p>\\n<!-- \\\/wp:paragraph -->\\n\\n<!-- wp:paragraph
